Care

Adult
Day Habilitation

We are committed to providing high quality services to our participant(s) in the comfort and safety of the center. Their individual needs are carefully assessed, understood and met through the selective assignment of qualified, trustworthy and compassionate personnel.

Consumer Directed Services​

To be a reputable vendor that assist consumers in hiring and training attendants to provide reliable and excellent care to enable consumers to have the support and assistance they need to enhance their independence.

In-Home Care: Agency Model

To receive Home and Community Based Services, In-Home Care: Agency Model the individual must be eligible and active with Missouri Medicaid. In-home care: agency model allows individuals to remain in the comfort of their own home.  The caregiver that performs services in the home is an employee of the agency when the individual chooses the agency model. Under the agency model, the individual can receive the following services…

– Personal Care: hygiene and bathing, grooming, dietary needs, toileting, basic transferring and medication prompting and reminding.
– Advanced Personal Care: ostomy care, transfer with assistive device, prescription medication application, catheter care, application of aseptic dressings, following a bowel program, passive range of motion and transferring with a device. A certified nurse assistant or an advanced personal care aide must perform these tasks.
– Homemaker: cleaning, laundry, changing linens, shopping/errands, taking out trash and essential correspondence.
– Respite: sitting with client; client must be at least 63 years old and qualify for the Aged and Disabled Waiver.
-Nurse Visits: medication set up, assessments and diabetic nail care

In-Home Care: Consumer Directed Services

To receive in-home care: Consumer Directed Services, the consumer must be eligible and active with Missouri Medicaid. The consumer must be at least 18 years of age and have a physical disability. The CDS program allows the attendant to provide assistance in the home, such as meal preparation, cleaning, and personal care.

The consumer will self-direct their own care, including hiring and supervising the attendant(employee) and approving time sheets. The attendant(employee) may be a friend or family member, at least 18 years of age, but cannot be a spouse or legal guardian.

The attendant(employee) must have a clean background check through the Missouri State Family Care Safety Registry before being employed by a CDS consumer.

Adult Day Habilitation Center

Adult Day Habilitation service is defined as the continuous care and supervision of disabled adults in a licensed adult day habilitation setting for up to 10 hours per day for a maximum of 5 days. Services include but are not limited to…

– assistance with activities of daily living
– planned group activities
– food services
– client observation
– skilled nursing services as specified in the plan of care
– community involvement

Planned group activities include socialization, recreation and cultural activities that stimulate the individual and help the client maintain optimal functioning. The Adult Day Habilitation Center does provide transportation to and from the center.

Forms of Payment

Missouri Medicaid is the primary payor for home and community based services.  Other options include Veteran Affairs (VA), Veterans Home Care (VHC), long term care insurance and private pay.

A1 Home Care opened and accepted its first client in November of 2011. It was opened by Amy Proffer RN BSN with a strong mission and vision in mind. Amy was then working for a local hospice agency and she began to see a huge need for a home care agency that offered weekend and 24 hour care to clients and families. It was very important for these clients to be able to stay at home and offer them a service that was affordable and available. She began the licensing process with the state of Missouri that took more than a year to complete.

Today A1 Home Care is a leader in the home care industry. It is a trusted agency within the community that has many programs and services to offer the clients and families they serve. A1 Home Care has grown significantly since opening in 2011. It employs/contracts 100+ employees and provides reliable home care services to over 150 clients/consumers.

OUR TEAM & OWNER

We are a dedicated team of professionals that are trained and directed to give the best possible care to our clients.

Most of the management team began their journey with A1 as a caregiver.  They understand the importance of great leadership and going above and beyond to assure the employees and clients are appreciated.

All employees are expected to have continued education at a minimum of monthly.  All the management team are expected to attend conferences, in-services and webinars routinely. 

A1 Home Care is not a franchise or nationwide agency.  The owner is involved in the business every day.  A1 Home Care is locally owned and operated by Amy Proffer RN BSN. Amy has worked for 25 years to build her reputation in the Parkland. Proffer graduated from North County High School in 1994 and completed an associate’s degree in nursing in 1997. Proffer then began her career as a registered nurse. Proffer earned her bachelor’s degree in nursing from Central Methodist University in 2009. In 2012, Proffer opened A1 Home Care. In 2015 she opened A1 Adult Day Care Center. Proffer is active with the Missouri Alliance for Home Care and Adult Day Care Association and works with the backpack program that donates to local school districts for children to have food when they go home. Proffer is currently serving in her fourth year on the Board of Directors for the Bonne Terre Chamber of Commerce, where she served as president in 2017. In 2015, Proffer received the “Business Leader of the Year” award from the Daily Journal and in 2017, she received the “Alumni” award at the Women’s Community Luncheon at Mineral Area College. In the same year, she was named “Citizen of the Year” by the Bonne Terre Chamber of Commerce. 

Proffer said her favorite quote is from Helen Proffer: “Character cannot be developed in ease and quiet. Only through experience of trial and suffering can the soul be strengthened, ambition inspired, and success achieved.”

OUR OWNER

Amy Keller has worked for 25 years to build her reputation in the Parkland and the hard work has finally come to fruition. Keller was chosen to receive the Mineral Area College 2019 Distinguished Alumnus Award.

Keller graduated from North County High School in 1994 and completed an associate’s degree in nursing in 1997. Keller then began her career as a registered nurse. Keller earned her bachelor’s degree in nursing from Central Methodist University in 2009.

Keller is the daughter of Daniel and Shari House, and Kathy and Tim Stanley. Growing up, Keller worked in her father’s business for many years. Keller also worked as a nurse on an as needed basis for seven years while managing three convenience stores.

Keller said that she was initially drawn to Mineral Area College because of the excellent reputation of the nursing program and the ability to complete a degree close to home. Keller added that she is grateful to MAC Nursing Program for advising her in all areas of her life. “The nursing program taught me how to organize, prioritize, and multi-task, all of which are crucial as a business owner and mother of three,” said Keller.

Qualifications: Must be 18 years of age or older and be able to pass a background check. This position requires transportation and a valid driver’s license. A1 performs all required training to be able to perform the duties on the current job description.

Qualifications: Must be 18 years of age or older and be able to pass a background check. This position requires transportation and a valid drivers license. Must hold a valid nurse aide certification through the state of Missouri or hold an APC certificate. A1 performs all the required training to be able to perform the duties on the current job description.

Can I receive home care services when I am on home health?
Yes, home care bills Medicaid & Home Health bills Medicare. Different services are performed in the home; therefore, they can coincide in the home.

What is difference between home care and home health?
Home care performs non-skilled services and bills Medicaid. Examples are personal care, nurse visits and respite services. Home Health bills Medicare and performs skilled services. Examples are wound care, IV therapy, physical therapy, occupational therapy or speech therapy.
